First Aid

All first aid related queries should be directed to our First Aid Officer who can be contacted on 8776 0200 or emailed at ashley.park.ps@education.vic.gov.au . If you require a form please refer below and complete, these can be emailed back to us or printed and brought in with the required medication and left in our medication basket at reception.

Medical Authority Form

All families please be aware that we are only able to administer medication to children as per the script direction from your GP or Paediatrician on the medication and it must be provided to us in the original packaging with the student’s name, method /time of dosage clearly labelled. Please note: we will not allow a student to take their first dose of a new medication at school in case of an allergic reaction. This should be done under the supervision of the student’s parents, carers or health practitioner

Asthma Management Plans

It is important that you check that the Asthma Management Plan you provide us with is the correct plan for your child’s medication; if your child uses a Bricanyl Turbuhaler, Symbicort Rapihaler or Symbicort Turbuhaler these will require a specific form (see below) to be completed by your GP /health practitioner or paediatrician as the recommended dosage may be different from first aid training for asthma. We also highly recommend that all children have a plan that includes the use of their puffer with a spacer and that you provide a spacer to the school to always keep onsite. All asthma medication must be kept in our First Aid office for your child not in their school bags.

Anaphylaxis Action Plans

Please ensure the plan for the appropriate autoinjector is provided as the use of an EpiPen or Anapen require different forms. This will need to be completed by your GP /health practitioner or paediatrician

Allergic Reactions /Allergies

For children who have allergies (not hay fever, refer to below section and allergic rhinitis form) the attached ASCIA Treatment Plan for Allergic Reactions will need to be completed and signed by your GP /health practitioner or paediatrician and returned to the school with the relevant medication/s. Please ensure that your GP /health practitioner or paediatrician list the required medication in this form so that a Medication Authority Form is not required.

Hay Fever – ‘Allergic Rhinitis’

For children who experience seasonal allergic rhinitis, also known as ‘hayfever’ or allergic rhinitis or perennial allergic rhinitis we would like families to have the attached ASCIA Treatment Plan for Allergic Rhinitis completed and signed by your local Pharmacist or GP and returned to the school with the relevant medication/s. This form will now supersede the Medication Authority Form required to administer these forms of medications.

For children who experience severe eczema please contact our first aid officer to consider if this action plan is required for any necessary management of the condition at school.

Out of School Hours Care

TeamKids are our OSHC provider.

TeamKids are our Out of Hours provider for Ashley Park Primary School.

TeamKids are proud to be independently owned and run by teachers.

Unique to the industry, TeamKids will employ a qualified full-time co-ordinator, known as a Director of Service, at our school.

The Director of Service has two overarching responsibilities, and that is to:

Implement age-appropriate experiences that are engaging and fun, but also create learning environments that are warm, unique and stimulating, a place where children don’t have to go, they want to go.
Ensure consistency of staff, provide an exemplary level of communication with the children and parents and build a strong, genuine relationship with the school and the broader community.

In addition to this, TeamKids have set a new standard in the OSHC space in their offerings that:

Build parental and student confidence through the quality of outstanding programs and communication.
Create child–led programs that enrich and extend the children in meaningful ways as their TeamKids Clubs.
Establish long-lasting relationships with our school by having complete community engagement, enhancing what already makes our school special.
Provide industry-leading educator-to-student ratios of 1:10.

All families will need to register for free with TeamKids. If you have not used TeamKids before then you will need to register your child and add your bookings.
